Code Inspections have been used very effectively at AT&T Bell Laboratories as part of the development process for 5ESS(R) Switch Software. They were first introduced to the 5ESS(R) Switch project in 1983. Beginning with a training program for all the developers involved in the project, code inspections have improved with each new 5ESS(R) Switch generic. This improvement is evidenced by a decreasing density of coding errors (number of errors per thousand lines of code) found during later phases in the software's life cycle, with each succeeding generic. The improvement in code inspections has been the result of hard work and innovation on the part of the 5ESS(R) Switch software developers, and use of some `Statistical Quality Control` (SQC) techniques.
