Intellectual Property Release Request (software program)

U-TRAC is a Lotus(R) 1-2-3(R) template designed to simplify the tracking of one's investment position in an employer co-sponsored employee benefit program configured as a savings plan. The template consists of nine individual forms which accept monthly data over a four-year period pertaining to the amount and direction of funding of the plan by the employee. The extent of employer dollar matching, if any, is also taken into account.
